IMPORTANT INFORMATION FOR NEW CLIENTS
Prior to your initial consultation, you will need to complete and sign a New Client Intake Form. This form is to be completed, signed and returned no less than 48 hours before your appointment. This ensures we have your correct details, including important information regarding any medications and allergies, etc., prior to commencing your consultation.

How to Book
To book your choice of either a face-to-face consultation or secure TeleHealth video consultation, it's easy. You can either:

I Need to Reschedule/Cancel My Appointment
Let's face it, even our best laid plans can go awry!
Initial Consultations (Face-to-Face and Online)
As your initial consultation time has been allocated especially for you, rescheduled, missed (no-show) or cancellations of less than 2 business days notice will incur 100% payment of the consultation fee unless, of course, there are extenuating circumstances. A timely rescheduling or cancellation ensures your vacated, extended time slot may be offered to a client on the appointment waiting list.
To avoid a situation of "double booking" yourself, simply forgetting your appointment or a having change of mind, we highly recommend you enter the appointment details into your diary or calendar. By way of courtesy, a reminder is forwarded to you via email 3 days prior and an SMS sent 24 hours prior to your upcoming appointment.

An invoice will be emailed to you 5 business days prior to your upcoming appointment. Please note that the invoice is to be paid, with proof of payment forwarded, no later than 2 business days (48 hours) prior to your upcoming appointment. Please note, non-payment of the invoice will equate to forfeit of your allotted appointment time, enabling it to be offered to a client on the appointment waiting list.
